DOT SAP evaluations by live video

Get back to work after a DOT drug or alcohol violation

I can schedule your SAP evaluation any day of the week, usually within 24 hours. If you're available, you can often be seen the same day you call. The evaluation is done by live video from anywhere in the United States, so there is no office to drive to.

DOT Substance Abuse Professional

For drivers and other safety-sensitive employees under 49 CFR Part 40.

A positive test or a refusal takes you off safety-sensitive work. You can't go back until a SAP evaluates you and you complete the plan. Here is how it goes with me.

  1. Initial evaluationA distance-based assessment by live video, with real-time audio and picture as Part 40 requires. Appointments run 7 AM to 7 PM, seven days a week, as times are open.
  2. Your planYou complete the education or treatment the evaluation calls for. I keep your employer's DER informed along the way.
  3. Follow-up evaluationWhen the plan is done, a second video evaluation determines whether you have complied. That finding is what clears you for your return-to-duty test.
  4. ReportsSent to your DER within 24 hours once the information is complete, with Clearinghouse reporting where it applies.

SAP evaluations are paid by you or your employer, with no insurance to wait on. Calls are returned within 4 hours, usually sooner. Every call and email with your employer about your case is included.

I see drivers by video anywhere in the United States.

Before you call: have your DER's name and phone number, the date of your test and a phone or computer with a camera.

An evaluation older than 90 days is no longer valid and has to be redone.

Can I do the evaluation from my truck?

Yes, if you can park somewhere private with a steady connection. The video and sound have to be clear enough for a full evaluation.

What happens after the follow-up evaluation?

You take a return-to-duty test. Once you're back at work, you'll have unannounced follow-up tests: at least six in your first 12 months.

My employer let me go. Do I still need a SAP?

Yes. The violation follows you to any DOT employer. Completing the process is how you clear it.

For couples

Relationship Rescue

For couples who keep having the same fight. The argument is rarely about the dishes. It's about what each of you has decided the dishes mean.

Relationship Rescue is a structured coaching program built on the cognition-first method I developed. You learn to catch the meaning you attach to what your partner does, test it against the facts and choose a better one.

Open to couples anywhere in the United States by live video, or in person in Jonesboro.

Precision Strategy Method Coaching

Advisory work for leaders facing a decision with real stakes. Readiness is never a feeling. It is a measurable configuration, and we build it before you commit.

Finish the cognitive work before you move.

Every engagement opens with a Discovery Conversation by phone. Call (870) 219-6312 or email CivilPsych@gmail.com to set one up.

Online, at your own pace

Dr. Heath's Mindset MasterClass

An online community on Skool where members practice the method together each week. It includes The 12 Ignition Cognitions practice series and Round Table, a live call every Thursday at 7 PM Central.

Coaching is not therapy. Coaching programs are not open to current or former therapy clients, and every coaching engagement starts with a short screening to confirm coaching is the right fit.

Therapy

Private pay. Individuals and couples in Arkansas and Missouri.

Thoughts come first. Feelings follow. What people walk in calling anxiety or depression usually turns out to be a way of thinking they've practiced for years.

We find the thinking behind the trouble, test it against the facts and practice a better one. You set the goals, and I teach you how to get there.

Individual therapy

Worry, low mood, anger and stress, and the habits that keep them going.

Couples therapy

For partners who want couples therapy with a licensed clinician. Looking for a set program instead? See Relationship Rescue.

Your first session

We talk about what brought you in and what you want to be different. You leave with a first piece of work to practice before the next visit.

Sessions are by secure video or in person in Jonesboro, by appointment. Therapy is open to clients who live in Arkansas or Missouri, where I am licensed.

Insurance: I also take Arkansas Blue Cross and Blue Shield (including Health Advantage and Blue Advantage), Medicare and UnitedHealthcare. Have a different plan? Call and I'll check it.
